Rule 9004. General Requirements of Form

  1. Legibility
    • All petitions, pleadings, schedules and other papers shall be clearly legible.

    With the growing number of ECF filers utilizing scanners, it is important to insure that the quality of your scanned document is legible.

  2. Caption
    • Each paper filed shall contain a caption setting forth the name of the court, the title of the case, the bankruptcy docket number, and a brief designation of the character of the paper.

    It is important when filing documents electronically that this rule be followed. If the pleading needs to be printed for any reason, the caption will play a major role in identifying the document.
