Code, Rules and Forms

There are several forms that must be presented when filing a bankruptcy case or pleading. Some forms are considered national forms (standardized forms) and some are local forms (customized for use in the Western District of Wisconsin). Filers are not provided with forms by court staff. It is important to ensure that the proper forms are filed in accordance with Federal and Local Rules.

NOTE: The following changes to the Federal Rules of Bankruptcy Procedure were adopted by the Supreme Court and are effective December 1, 2017: Bankruptcy Rules 1001, 1006, 1015, 2002, 3002, 3007, 3012, 3015, 3015.1 (new), 4003, 5009, 7001, and 9009.
