Note:
Certificate of Credit Counseling must have been issued during
the 180-day period ending on the date of the filing of the petition.
All individual debtors must file (WITH
THE PETITION) Exhibit D - Individual Debtor's
Statement of Compliance with Credit Counseling Requirements
(Official Form 1, Exhibit D) which must include one of the following:
an attached Certificate
of Credit Counseling, or a
statement that the
debtor has received the credit counseling briefing but does
not have the certificate, or a
