Certificate of Credit Counseling

Note: Certificate of Credit Counseling must have been issued during the 180-day period ending on the date of the filing of the petition.

All individual debtors must file (WITH THE PETITION) Exhibit D - Individual Debtor's Statement of Compliance with Credit Counseling Requirements (Official Form 1, Exhibit D) which must include one of the following:

1. an attached Certificate of Credit Counseling, or a

2. statement that the debtor has received the credit counseling briefing but does not have the certificate, or a
   
   Note: The Certificate is due within 14 days.

3.  § 109(h)(3) Certification re: Exigent circumstances, (Exhibit D may serve as the certification if an explanation of the exigent circumstances is included), or a

4.  § 109(h)(4) Request for Exemption from Credit Counseling.

To File Certificate Electronically:

1. Log into CM/ECF.

2. Select [Bankruptcy >  Miscellaneous].

3. Enter the case number (e.g, xx-xxxxx).

4. Select [Certificate of Credit Counseling] from the event list.

5. Skip the Joint Filing screen.

6. Select the party filer.

7. Browse to select the Certificate of Credit Counseling (pdf file).

8. Select Debtor, Joint Debtor, or Both for drop-down list.

9. Verify the final docket text; if correct, click [Next] to submit your document(s).
   
   Note: The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
