Proofs of Claim

Filing Requirements:

  • Proof of Claim (must conform to the official Proof of Claim form, i.e., contains required information)

  • Indicate a signature like "/s/ Jane Doe," unless the document has been scanned and shows the original signature.

To electronically file the claim:

  1. Complete Proof of Claim. Proof of Claim form (pdf)

  2. Scan/save the entire claim as a pdf.

  3. Log into CM/ECF.

  4. Select [Bankruptcy >  File Claims]

  5. Enter the case number (e.g., xx-xxxxx) [Find This Case] Next.

  6. Search for the creditor, with an address matching the notice address, from the drop down list.  Verify that the creditor's address is the same. If creditor is not listed or has different address, Add Creditor.

  7. Enter Proof of Claim information:

    • Amends Claim #  Clicking on [Find] presents any previously filed claims.  Highlight/click on the claim being amended, confirm and continue.

    • Filed By: Select appropriate Party Filer, (e.g., Debtor, Attorney, Creditor, Trustee)

    • Amount Claimed: Enter amount claimed (do not enter "$" or commas in the dollar amount fields) Tab for automatic calculation of total claim amount.

    • Description or Remarks: Type further description or remarks for the claim.

    • Select appropriate radio button for Amend Options:  Clear Amounts, Description, and Remarks; Clear Description/Remarks; Clear All Amounts.

  8. Verify all claim information entered.

  9. Browse to select the Claim (.pdf file).

    Note:  Exhibits/attachments to the claim may be added as attachments to this document.

  10. The Notice of Electronic Filing displays giving you the document number. Copies of this notice are immediately e-mailed to all participants who receive electronic notification in the case.
