New Chapter 13 Plan Form and New CM/ECF Event with Corresponding Local Form, Effective November 15, 2023

The United States Bankruptcy Court for the Western District of Wisconsin, in collaboration with Office of the Chapter 13 Trustee and the Local Rules Advisory Committee, have revised the Western District’s chapter 13 plan form and approved a new local form, Notification of Change in the Manner of Payment to the Chapter 13 Trustee.

Beginning November 15, 2023, all chapter 13 plans, including initial, amended, and modified, must conform to the new chapter 13 plan form, pursuant to Local Rule 3015-1. The new chapter 13 plan form is available on the court’s website.

NEW Chapter 13 Plan FORM

Deficiency notices may be sent out in response to the use of the old form after November 15, 2023.

The new local form, Notification of Change in the Manner of Payment to the Chapter 13 Trustee, is also effective November 15, 2023. This form has a corresponding new CM/ECF event – Notification of Change in the Manner of Payment to the Chapter 13 Trustee.

The new event is located under Bankruptcy Events > miscellaneous and bankruptcy events > Trustee/US Trustee menu options.

The purpose of the form is to provide debtors a method to notify the court to a change in the manner of payment to the chapter 13 trustee, in instances such as a change of employment. This form will also help clarify requests for amended wage orders.
