Forms

There are several forms that must be presented when filing a bankruptcy case or pleading. Some forms are considered national forms (standardized forms) and some are local forms (customized for use in the Western District of Wisconsin). Court staff does not provide filers with forms. It is important to ensure that the proper forms are filed in accordance with Federal and Local Rules.
